If your child goes to OCD aftercare, please complete form and email to ocdmorningandaftercare@ccusd.org or turn in to OCD office (NOT SPOTLIGHT) before starting classes. A new one must be completed before each session. (This is not for CCARP, Iverbe or YSE)
